Coventry SEND regularly requests feedback from parent/carers, children, young people and professionals across the Coventry area to review the support children and young people (0-25yrs) with special educational needs and disabilities are offered.
As well as identifying and reviewing what is already available across education, health and social care, it is also there to help identify any improvements needed or gaps in provision.
We want to give you the opportunity to tell us what you think and ask any questions that you have. This will help us to identify the sort of provision in Coventry which will benefit our children and young people the most.
Have you signed up to the half termly “In Partnership” e-bulletin from the SEND Information Advice and Support Service? This provides information about SEND both in Coventry, including upcoming events and changes to services, and national stories.
You can sign up through the City Council website, just select “Coventry SEND news” under the Children and Families section.

As part of Coventry's assessment of how children and young people with SEND are progressing and being supported, we would like to hear about their experiences, as well as those who support them. This survey looks at a few main areas which, in working together with families and professionals across the city, we can build upon to help Coventry's "Local Offer" for those with Special Educational Needs and Disabilities.
Children and Young People with SEND covers a wide range of areas and services so this survey may not directly cover something that you feel needs to be raised. If this is the case, then please put these thoughts under the questions which offer this option.
If you have more than one child with SEND, please feel free to complete the survey for them too - please complete one survey per child.
Coventry's Parent Carer Forum are looking for increased engagement and participation amongst parents and carers in the city, and the results of this survey will be shared with them to identify ways to maximise their voice. This form is for providing feedback on your experiences of using SEND services in Coventry. It can be completed by young people themselves, along with their parents and carers, and the professionals working with them. Where possible, detailing your answers will help not only to describe your experience, but also help us understand what works best for young people with SEND in Coventry.
The information you provide will be shared with the people who lead Coventry’s SEND Services and be used to improve the way we work with children and young people with SEND. On occasion, we may also:
Share information with Health, Care and Education professionals to help them understand the experiences that children, young people and families have of the SEND system.
Include information in our self-evaluation to show the changes we’ve made to SEND services and why.
This survey should take no more than 5 minutes to complete and will be hugely valuable in identifying next steps to support school leaders in this area.
