Dementia Hub Recognising a Dementia Hero - Nomination Form

During the COVID 19 pandemic many services were reduced or closed, while others had to adapt the way they provide services in order to continue to support vulnerable people.

The Council run Maymorn Centre in Holbrooks was at the time a Monday to Friday day service for people with advanced Dementia - demand for the service had reduced and this had to close due to the vulnerability of clients, it was felt that the service should be reviewed during this closure to ensure it best meets the needs of people living with dementia and their carers.

As a result and working with the Alzheimer's Society and a consultation it was decided to change how we support people with dementia, as well as providing outreach services it was recommended we redevelop the building to provide a Dementia Hub

A single hub as the ‘go to’ place for people with dementia and their carers. The hub would:

  • be a ‘one stop’ shop for advice and support, to reconnect with others
  • be a base for a number of organisations involved in providing a service to people with dementia
  • provide services on and off site.
  • support a much wider cohort of people with dementia, from younger adults/early onset dementia to those with memory issues without a diagnosis of dementia.
  • provide a social space for people to meet and interact based around a café


 We are very excited by the changes ahead and recognise that the journey to improve lives for people with dementia and their carers has been advanced by many passionate people! We recognise that some people have dedicated their lives to the cause both past and present and, we hope to identify those who have made a real difference.

As part of the vision to create the Coventry Dementia partnership Hub  we are seeking nominations

We want to learn about, respect, and honour those who have made it their life work to improve the lives of those living with dementia.  

Do you know someone who has worked tirelessly to improve the lives of those with dementia? Someone whose lifelong goal was to make a difference? Someone whose passion and drive have made a change?  

Please tell us about them. 

This person may still be with us or could be someone who has passed away.  

The Dementia Hub will host community rooms, allowing the wider community to benefit from the hub too. We would like to name the community rooms in honour of some of those whose contributions should be recognised. 

The information you provide will only be used for processing the Recognising a Dementia Hero scheme.

Your details will not be shared with any third party, and will be used solely by Coventry City Council’s Adult Social Care Team, and only with those directly involved in the processing of nominations and the room naming decision.   

Contact details of the nominee(s) will only be used to gain permissions from the intended recipient or their next of kin where necessary.

The decision will be made by an external panel of 3 people.
