The purpose of this pre screening questionnaire is to determine whether it is appropriate to carry out a visit or inspection of premises.
The safety of our employees, their families, partners, and customers families are the Council’s overriding priority. As the coronavirus pandemic continues, the Council are monitoring the situation closely and will periodically update guidance based on the current recommendations provided by Government.
To prevent the spread of COVID-19 and reduce the potential risk of exposure to our workforce and customers, we are conducting a simple screening questionnaire. Your participation is important to help us take precautionary measures to protect everyone during this time.
